Pregnancy in a Obese Married Lady with Hyperprolactinemia, Severe Hypertension Mildly Raised ACTH with Symptomatic Treatment Like Topiramate, Caberlin, Labetolololand Cycle Correction-Live Healthy Baby

A 29 year old patient married for 5 years presented with irregular cycles, obesity and primary infertility of 5 years. On examination she was hypertensive with BP 190/110mm HG, Ht- 180cm, Wt-120kg BMI, 37.3Kg/m2 SFSH- 6.2iu/l, LH- 9.2IU/L, S prolactin- 191ng/ml (5-29ng/ml), Thyroid function test was normal, s.cortisol-5(wnl), SDHES-WNL, GTT-within normal limits, postprandial insulin 56u-wnl, s testosterone was 0.6ng/ ml (0.2- 0.8ng/ml). ACTH was increased to 56pu/ml. Hence low dose and high dose dexamethasone suppression was done which was within normal limits.

Ruptured Rudimentary Horn Pregnancy in a Multiparous Patient: A Case Report and Literature Review

She presented with a rupture of undiagnosed rudimentary horn of uterus at 18 weeks. Laparotomy was performed and rudimentary horn was excised with good recovery postoperatively. The need for high index of suspicion and early diagnosis using ultrasonography is emphasised. Conclusion: Pregnancy in a rudimentary horn carries grave risk to the mother. There is need for increase awareness of this rare condition and to have a high index of suspicion at time of booking. Global Journal of Reproductive Medicine   Juniper Publishers Abstract The cryopreservation procedure changes the integrity and functionality of sperm, affecting their longevity and the ability to fertilize the oocyte. Moreover, some of those changes could be alter the physiologic interactions between the sperm and the oviductal epithelial cells prior to fertilization, placing further limitations and challenges on frozen-thawed stallion sperm. For fertilization to occur, a sperm subpopulation should be survive in uterine environment and be transported to oviduct, the tubular epithelium that regulate the sperm capacitation process in tuning with the ovulation and later oocyte fecundation.
